Failure to submit your completed Claim Form on time by U.S. Mail (properly addressed) or fill out an online Claim Form by the deadline will result in the rejection of your Claim and you will not receive any money from the Settlements.

For purposes of these Settlements, a direct purchaser is a person or business who bought certain CPPs directly from one or more of the Defendants, co-conspirators, affiliates, or subsidiaries themselves, as opposed to an intermediary (such as a retail store).

For purchases during the Class Period for each individual Defendant, identify the CPPs purchased within the United States, including its territories and the District of Columbia, directly from one or more the Defendants, or any of their predecessors, subsidiaries, or affiliates thereof, and all federal or state government entities or agencies, the year(s) of your purchase(s) and the total dollar amounts:
